Cancellation Policy
We are committed to
exceptional customer service and
clinical care to
expedite the healing and recovery process.
To accomplish this, it is extremely
important that you attend each of
your scheduled appointments. If you know
that you will be unable to make a
scheduled appointment,
call our Front
Desk Coordinator immediately for
rescheduling, and allow us to fill your
therapist's time slot.

We
require 24 hours advance notice of
appointment cancellation

In the
event of a late cancellation or
"no-show," your account will be
assessed a $20 cancellation fee
-
Three cancellations or no-shows during
the course of therapy requires your
therapist to discharge you from
therapy
-
Workers' Compensation patients are
not charged for cancellations or
no-shows, however we are required to
notify the patient's Physician, Case
Manager and Employer of
non-compliance with therapy

Payment Options
You are
financially responsible for all services
provided in the event your insurance
carrier declines payment.
Payment of
co-payment and unmet deductible
is expected at the time of service. We accept MasterCard,
Visa, personal checks, money orders or
cash. We also offer "Easy Pay," where
your credit card is kept on file and
automatically deducted after service.
Returned
checks are subject to a $25 processing
fee. We reserve the right to pursue
the California "Bad Check Law" Civil
Code B719.
Insurance Issues
We accept most forms of private
insurance, Worker's Compensation Insurance, and Medicare. Please see our section about
insurance.
